Patient Portal
Welcome to your medical home on the Web. With the Family Practice Group Patient Portal you can connect with your clinic through a convenient, safe, and secure environment from the privacy of your home or any other location with an internet connection. Through this portal you can:
- Request an appointment with your primary care provider
- Request medication refills
- Access your health record
- Exchange secure messaging with your doctor
How To Sign Up:
1- For your security, please download and complete the appropriate form below. When completed, you may either mail the form to Family Practice Group or bring it into the clinic at your earliest convenience. Mailing address: Family Practice Group, 229 Stewart Avenue, Medford, Oregon 97501
- Patient Portal Agreement for an INDIVIDUAL Portal Account (for an individual who does not share an email address).
- Patient Portal Agreement for a FAMILY Portal Account (for family members who share a common email address). Please note access to the account enables the viewing of all family member medical records within the same Family account.
If you have questions regarding which form to fill out please contact our office at 541-779-5531 and a staff member will assist you.
2- During your next office visit you will receive an activation code. You may also receive the code over the phone once we have received your Patient Portal Agreement via mail by calling 541-779-5531. The code will be emailed to you via our secure system. This code allows you to log on and enroll to create your new online patient account, user ID and password.
How to Log In:
Once you have enrolled for the Patient Portal, you will be able to log in to your account with your user ID and password.
